Forms

For all financial aid forms, please go to the  Financial Aid Online. You will log in with your CIU Online username and password. When you log in, click on the "documents" tab to retrieve the forms you need. Make sure that you are logged in to the correct school year, as the forms change from year to year. You can change the school year at the top of the home page of the CIU Online Financial Aid System. To view all the forms click on the "forms" tab in financial aid online.
